I have warranty on my phone but my dad did all of the paper work, he passed away in October and I have no idea how to do this. My phone doesn't charge and my lock button is broken. Also would I be able to change the color of my iPhone to black? I have the white one right now.

iPhone 5 Sleep/Wake Button Replacement Program - Apple Support
you cannot change the color of the phone. You would need to buy another device.
Also I don't understand the paperwork was done by my dad statement. Is the device and account in his name? If so someone else must contact Verizon, possibly the executor of his estate.
http://www.phoneclaim.com if you have the Asurion supplemental insurance.
If just under a year, you can go to any Apple store for warranty service or call 1-800-922-0204 and Verizon will send you a second hand device to replace the phone if there is no water damage or physical damage to the device.
Good Luck
