You can call in to Customer Care to make the change, they can do it at a store, or you can do it online.
To do it online, go to the Verizon Wireless website, then from the menu bar at the top, select My Verizon > My Devices > Activate or Switch Device. Then select "Activate a Device on an Existing Line," pick the phone number you want to move, and it will walk you through the process from there.
