safe mode puk locked out

My phone is in safe mode. It is puk locked out. Please help. I can't even do a factory rest because it's locked. The phone is not active with any service provider.

Somehow I missed the "not in service with any provider" part.  You'd need account owner status and an active line with Verizon to do this.
