iPhone 5 camera won't autofocus

I have had my iPhone 5 since it was released in October 2012. Around 3 months ago I noticed the camera would not autofocus when I would go to take a picture (especially of up close font). The problem gradually started to worsen to that point that I now have to either flick the camera lens or lightly smack the camera on the palm of my hand in order for it to focus. My phone has not had any out of the ordinary drops.---No damage is physically on my phone, no huge dents or broken glass. Restarting my phone does not help the problem at all, as soon as i operate the camera again it will still not focus. Is there any way to fix this problem or will Verizon supply me with a new phone due to the fact that the lens keeps sticking?
