Oh my, I hope you're not experiencing this issue any longer! If so you can obtain your PUK code by following these steps:
Log into vzw.com and choose the mobile number in question. Under "I want to..." click on the category Device. Then choose Unlock SIM (PUK) Now. The following screen will allow you to enter your PUK code.
Generally when you try to place a phone call it will prompt you for the PUK code again where you can enter it if you lose the screen.

The issue has be resolved. I ended up calling last Monday. The Support person talked me through a lot of stuff and she had to do a lot of stuff on her end. I am just glad it is fixed. Thank you for replying.
