Have you made sure to disable the auto settings and app option on restore that has a bad habit of restoring bad settings after restore that make issue continue.....
yes ive done this along with both methods of the factory reset ive gone as far as deleting all apps before restoring also and still nothing has improved?
Sounds like you may have a hardware issue, I would have tech look at device and see if they have a solution because usually this issue is related to the programming, rebooting, hard resetting or reprograming usually resolves this issue.
You have covered two of the three, try dialing *228 and #1 and see if this helps. If not, off to the store I would go...
