You need a code to open the PUK. I suggest that you take a trip to your local VZW store and they can help you. Don't try to mess around with the phone because you just may lock yourself out permanently.

To get the PUK unlock code, you can phone Verizon Global Services, (800) 711-8300. I would suggest this rather than visiting a store.
But, first some questions! Are you in the US? If so, does you phone work in CDMA mode? The SIM lock should only be an issue in GSM mode. Have you tried removing the sim card from the phone? The display will then warn you that no SIM card is present but should work fine. (If you are out of the US and need GSM, that's obviously not a solution!)
Also, to get to this state, you would normally need to have enabled SIM security (off by default) and then entered the wrong code a number of times. Did you do this? If not, there may be something wrong with the phone. Try getting it unlocked anyway, but you may need to then take it to a store if the problem continues. (If you did do this, try not to do it again after getting the unlock code!)
