How do you check your upgrade eligibility? How often can you upgrade? I was told once every twelve months when I signed my contract but when I called in I was told I couldnt until the two years was up.

You can't upgrade unless the contract term is completed or you have paid the device payment agreement in full either over 24 months or if you pay off the remaining balance early.

My Verizon. It tells you which devices are upgrade eligible.
iPhones when purchased new have special financing that let you do a yearly upgrade with trade-in. So long as your device is in good working and cosmetic condition.
Other phones (outside a very small handful of Samsung units) have the full 24th commitment where you have to pay the agreement off 100% to upgrade.
