call 611 and press the option to report a lost or stolen phone or no service

somnia, I'm not sure what exact number you're looking for; however, as Kev48us mentioned, you can always call Customer Service at 800-922-0204, or you can dial *611 [send] from your mobile phone. 
 
You can contact Roadside Assistance at 877-623-7433 or you can dial #ROAD [send] from your mobile phone.
 
If you need to reach Asurion, you can call them directly at 888-881-2622 or online at the following link: Asurion.
