Need a PUK pin for an deactivated iPhone 4s

My wife had an iPhone 4s.  She got a new phone because it was having issues connecting to the internet and the battery not lasting.  

NOW the Sim Pin is locked and we need a PUK code.  The iPhone 4s is no longer in my list of devices and the chat agent was useless.   How can I get this PUK code from. We want to give the phone to my granddaughter

I do understand your concern to get to use your device. You can try our basic passwords for most PUK 00000 or 11111. Let us know if the codes work. We want you to have access to your devices.
