I have the same problem. I accidently locked my SIM and can't figure out where to find my PUK code. I think all I need is directions on how to find it so I can enter it.

Locating the 4G SIM PIN / PUK - My Verizon Website
- Navigate to the Verizon Wireless website, enter the 10-digit mobile number and password then click Sign In.
To register, click Register then follow the step-by-step instructions.
- Navigate My Verizon > View All (under My Device).
- Click Unlock SIM (PUK) Now (under Device Actions).
- The default PIN and PUK key are displayed.

I apologize for breaking the rules and why they exist. I have a printout of
the steps that are required to successfully solve my Sim lockout. To my
embarrassment, going step by step up to seeing my entries
totally block me. I checked on My Verizon, as required and finally got in
touch with the SIM Verizon Staff people. The first gave me the Sim number,
minus the pass word, it didn't work. The "Global desk person tried over and
over to unlock the Sim Code, no luck. The last thing she told me was she
was sending a new SIM card, which should be here on this coming Tuesday.
Who knows whether it will work. If not I will just pull the new card and
give up. I would like to know why my persuit through for the completion on
the website was frozen to me, even the times I used My Verizon never froze.
Once more, please accept my apology for breaking the rules.

You may want to look into removing your personal email from your post to avoid the moderator having to edit all your post... This is to protect your personal information and any possible issue with users having this information.
If this is being automatically added with your post you should be able to change this at https://community.verizonwireless.com/edit-profile-security!input.jspa?targetUser=9292 , simply change email option to "Yourself".
