You must be a consumer account,
Monthly access of $79.99 or higher for single and Family SharePlans (for family share plans this means the primary line on the account must be 79.99 or higher not that the total for the multiple lines is over 79.99 or if there is a separate single line that is 79.99 or higher that'd work too).
Must have been on the 79.99+ plan for 6 months or longer
Has to have had service for 12 months or longer
You cannot be past due 60+ days
This is supposedly reviewed annually so you can lose your status.
