Where can I find the link to the Switchover Promo?

I recently switched from my family's Sprint plan over to my own line on Verizon. When I asked about the switchover promo ($650 per line) I was told by an in-store rep (not a corporate store) that I could initiate the process online on MyVerizon. I'm unable to find the link and I had lost the paper the guy had given to me. Can anyone help me out?
