Phone insurance

Dear verizon,  if I want to buy phone insurance or add it to my home owners insurance. ....how do I obtain proof of purchase if I got the phone by using the program where I make monthly payments on the phone being added to my phone bill every month? No receipt is given for that type of transaction!
