I received a claim form in the mail in September, advising me that I hadn't cashed a check that was sent to me in 2008. The letter advised me to fill out the claims form and send it in by Sept. 21st, and that I should receive my credit refund check in 6-8 weeks. I assumed that it was regarding a security deposit that I paid when I was an Alltel customer in the military.
It's been over two months now since I sent the form in, and I still haven't received any further communication about this. I don't know who to contact about it, and I'm not even 100% sure that it was legit. I would really like to know what's going on with this. Am I still eligible for the refund? Has the check been mailed already? Did my form get lost?
Does anyone have any kind of advice on how to deal with this?

What kind of information did you have to fill in on the form you mailed?
If it was any important personal information, I would be sure to routinely check your credit in case someone has opened any new accounts in your name.

I only had to fill out my name and address, and sign the form, then send it back.
