How does one purchase a phone in a Verizon Wireless store?  Is it by swiping or sliding a debit or credit card, or the phone gets added to a line and the money gets taken from the account that is paying for the phone services?

If you are on post-pay and you go into a store I would suggest buying using the DPP. You will pay the tax on the phone and monthly payments will be billed to your account.

If a phone has a no contract price of $100 and a monthly payment price of $4, and the customer chooses monthly payment price of $4, then how much would the customer have to pay when purchasing the phone?

The monthly payment price is simply the no contract price divided by 24.

If the no contract price is $100, then the monthly price would be $4.17 and the total price paid would be $100 after 24 monthly payments.

The customer would have to pay the tax on the phone at the time of signing the DPP agreement.

How much is the tax on a phone, is it the no contract price x 0.0825%, if not then what is it?

The sales tax is based on the full retail price (no contract price) multiplied by the Sales Tax for the location of the store or your zip code if ordering online.
