Sometimes it is the littlest thing which can generate this type of message.
For example, I have a couple of phones I trade back and forth since I keep changing my mind about which device I would like to use. Each time I take the SIM card out of one phone and place it in a different one, I get this message. There is nothing which I did online, over the phone, or in a store. I simply pulled the SIM card out of 1 phone and placed it in a different phone.
I am unsure what other type of actions can trigger this message, but sometimes it can be the littlest thing which you wouldn't necessarily think should generate this type of message.

I've received them when I've added a number to the five free blocked numbers list. I wouldn't consider that a feature change, but apparently Verizon does.
