I need to obtain PUK from older phone that I can't recall phone number or pattern

Need PUK for older phone. I went to local store and was told to talk to someone at Samsung. But it has a SIM Card in side which I thought was from verizon

We know how important it is to get the device details and want to provide you with the correct expectations, the PUK code is related to the SIM card, and the only way we can request or retrieve the PUK code is by having the SIM card activated. You mentioned that is an old phone, are you wanting to activate the phone on a current line?
