I am often looking at my phone and see that the camera app has accidentally started. This is a Galaxy Note 3 running Android 5. I know there are several "quick" ways ithe app can be started. What might I be doing accidentally and how can I disable this feature?

The shortcut options are definitely very helpful but we want to save the room on your device for pictures that you are aware is being captured. We will check out a few things to see if we can pinpoint this issue for you. Is your phone locked when you notice that the screen is on camera mode? Do you notice if the phones volume or power buttons are being pressed as well by accident?
