After working with Tier 2 Support for several days, it was determined that my Verizon LTE Network Extender was working properly and that the issue must be the phone.
Resets and Factory Resets did not fix the problem nor configuration changes.
Eventually, Verizon replaced the phone and the new unit has functioned properly and stays connected to the extender.
