The Google Pixel 4 XL does not have a 'Pulse Notification Light' option, unlike some of its earlier versions. Do you recall adding/downloading a third-party app? Can you place your phone in 'Safe Mode' (https://www.verizonwireless.com/support/knowledge-base-228678/) and test it out (to see if it flashes during a call)?

My Pixel 4XL started flashing on call again after clearing the Phone app storage. It turns out a third party app "Prof reminder" (Missed call reminder, Flash on call) was carried over from my previous Pixel. This app has missed call and SMS reminders but also has a "Flash on call" function. Once I disabled the Flash on call function it stopped flashing.
On the other hand, if you want your phone to flash on call this app will add this feature to the phone.
