How can I fix/diagnose constant loss of 4G connectivity?

Over the past couple of weeks I've had next to zero 4G LTE connectivity. My area has strong 4G coverage, and others around me with Verizon are not seeing similar issues. When I reset my Bionic or toggle airplane mode I may get a 4G signal momentarily, but even in that case it drops to a 3G connection within minutes. Under the diagnostic screens, I've observed that the device is set to "LTE/CDMA/EvDo" as the preferred network type. The last major change to the device was a factory reset approximately 3 months ago in an attempt to resolve performance issues. I have not made any recent changes to my setup, changed regions, or caused any damage (impact or water) to the device, the 4G SIM has not been removed and is firmly seated.


Other than walking in to a Verizon store and upgrading to a new device, what are my options here? What data can I provide to aid with a diagnosis/fix?
