Unlocking Phone and Initial Programming

So my phone stopped working today out of random and now it's saying "initial programming needed". I see *288 well let you reset the phone, but my phone won't let me into it so it can be reset. It says my secerity code doesn't work, but i haven't changed it or reset it at all. What can i do to get back into my  phone?

Unlock code should be last 4 digits of phone number. If you don't know the key for the phone and it's NOT programmed choose emergency call dial *228 and program/activate phone and the key will change to last 4 of phone number.

You will have to go into a corporate store and have the phone flashed.  You can locate the closest store here.  Thank you.
