Why are all incoming calls showing up as "Unknown"?

Switched from T-Mobile to Verizon. All incoming calls show up as "Unknown", even from people on my contacts lists. Not only do they show up as Unknown, but their number doesn't show up! What do I do?

I am having a similiar issue. Everything was fine until a couple weeks ago. Now the number shows up, but it doesn't show my contact name for that number.

We don't want you to have to play guess who every time your phone rings, whitejw. What device do you have? Does this affect all calls?

It is an HTC6500LVW.

Yes all incoming calls show up as "Unknown".

We certainly want to make sure you know who is calling jms2105. Let's make sure we get this working for you. Has this been happening since the first day you switched to Verizon or did it start some time after? Do your text messages come through showing the contact name or number?

I have had the same issue and it has happened for every incoming call since I got the phone two weeks ago. I got a new iPhone 6s for work. Any ideas?
